Respite Service

Our free respite service enables Carers living in the London Borough of Hounslow to have a break from their caring role by providing paid, trained Care Support Workers to take over the caring role on a regular basis, actively working with people with disabilities to enable them to maintain their independence wherever possible.

The service is funded by the Hounslow Primary Care Trust, Hounslow Social Services and donations.

Hounslow Crossroads care is individually tailored to meet the needs of each family and is flexible and adaptable around their needs, rather than families having to fit their needs around the availability of the service.

Individual care plans are agreed between Hounslow Crossroads, the Carer and the person with care needs, respecting lifestyles, personal choice and cultures. People are able to decide for themselves what help and support they need.

Care Support Workers are matched to individuals needs and where possible, the same Care Support Worker will be provided on every visit.

By providing this valuable type of respite service at home, our service enables Carers to continue caring for as long as they wish or are able to do so. Our sensitive and flexible approach is designed to help Carers and the people they care for feel comfortable and in control of the care that they receive.

Carers can use the time they receive from Hounslow Crossroads to go shopping, see friends, attend important medical appointments or even just catch up on sleep.

When the service can be offered, the Carer and person with care needs will be visited by the Service Manager of Hounslow Crossroads to carry out a full assessment and discuss how our service can meet their needs.

Our service works closely with health professionals, social services and other voluntary organisations to ensure the needs of Carers and the people they care for are met.

How to access the Respite Service

Referrals can be made by anyone, including the Carer, friends, neighbours, GPs, District Nurses, voluntary organisations, or the local authority by contacting our office on 020 8572 8834.

 

Macmillan Crossroads Palliative Care Service

Our free Palliative Care Service supports Carers living in the London Borough of Hounslow looking after someone with a diagnosis of cancer or other life-limiting illness.

This service is funded by Macmillan Cancer Support to help Carers manage the day to day care, allowing the person they look after to remain in their own home for as long as possible.

Our service provides paid, specialist Palliative Care Support Workers to take over the caring role at times arranged flexibly to meet the needs of the Carer and person they care for.

Individual care plans are agreed between Hounslow Crossroads, the Carer and the person with care needs, respecting people’s dignity, culture, personal choice and lifestyle.

The Palliative Care Support Workers provide assistance with all aspects or personal care for the person with care needs, offer emotional support, provide meals, and other practical support to enable the person with care needs to engage in activities.

This time enables Carers to go shopping, see friends, attend important medical appointments, catch up on sleep or do things that provide them with time for themselves.

Palliative Care Support Workers are matched to individuals needs and where possible, the same Care Support Worker will be provided on every visit.

We aim to contact families within 48 hours to carry out a full assessment and discuss how our service can meet their needs.

Lyn Hammond is the Carers Rights Worker for Hounslow and can be contacted on 0208 570 6963.

Lyn runs a carers desk on a Wednesday afternoon from 1pm at West Middlesex Hospital at the main reception. No appointment is required.
